The integrated Care Connect solution for remote follow-up of people with Type 1 Diabetes advances throughout Europe, leveraging the Caaring® digital platform

The Care Connect solution, focused on comprehensive care for people with Type 1 Diabetes, continues to expand, helping to optimize the healthcare process and improve health outcomes and experience of patients using continuous insulin infusion systems.
The initiative was first implemented in Extremadura through the Extremadura Health Services (SES in Spanish), thanks to Medtronic Ibérica and technologically developed by Persei vivarium. It is achieving promising results in that community, as it is already being used by more than 750 patients. All of this has allowed the project to expand to other autonomous communities in Spain and to other countries in Europe with the support of Medtronic, where they hope to replicate its success.
The Care Connect solution is based on a digital platform and some services aimed at facilitating the adoption of technology within the hospital, as well as reducing the administrative tasks carried out in the center, inherent to remote patient monitoring. In this way, Care Connect facilitates remote monitoring of people with Type 1 Diabetes, helping healthcare professionals optimize and prioritize care according to their progress. Likewise, the solution includes a training program that promotes patients’ empowerment and self-management of the disease. Patients receive educational material through the mobile application Caaring®, which helps them better understand their condition, therapy, and achieve a better quality of life.
Currently, the project is underway in 21 hospitals of various countries in Europe and the Middle East, and it is expected to continue growing in these regions with the aim of delivering high-value outcomes to patients, healthcare professionals, and hospitals.
In addition, it is hoped that the project will continue expanding in Spain, optimizing the management of this type of patient, and promoting their self-management through technology, with the intention of being able to encourage and achieve the efficient use of healthcare resources.
