The<\/span> Care Connect<\/span><\/a> solution, focused on <\/span>comprehensive care for people with Type 1 Diabetes<\/b>, continues to expand, helping to <\/span>optimize the healthcare process and improve health outcomes and experience of patients using continuous insulin infusion systems.<\/b><\/span><\/p>\n The initiative was first implemented in<\/span> Extremadura through the <\/span>Extremadura Health Services (SES in Spanish)<\/span><\/a>, <\/span>thanks to<\/b><\/span> Medtronic Ib\u00e9rica<\/a> and technologically developed by<\/span> Persei vivarium<\/a>. It is achieving promising results in that community, as it is already being used by <\/span><\/span>more than 750 patients<\/b>. All of this has allowed the project to expand to other <\/span>autonomous communities in Spain and to other countries in Europe<\/b> with the support of <\/span><\/span>Medtronic<\/a>, where they hope to replicate its success.<\/span><\/span><\/p>\n The Care Connect solution is based on a<\/span> digital platform and some services aimed at facilitating the adoption of technology within the hospital, as well as reducing the administrative tasks<\/b> carried out in the center, inherent to remote patient monitoring. In this way, Care Connect <\/span>facilitates remote monitoring of people with Type 1 Diabetes, helping healthcare professionals optimize and prioritize care according to their progress.<\/b> Likewise, the solution includes a <\/span>training program that promotes patients’ empowerment and self-management of the disease<\/b>.
